My office operates on a fee-for-service basis. Payment in full is due at the time of service. I accept payment in cash, check or by credit card via a patient portal site.
FEES
Individual therapy $165 - 50 minutes
Couples/Families $200 for 60 minute session $225 for 75 minute session

Please read and sign the following forms prior to your first visit. Alternately, I can give you a copy and you can review and sign in your meeting with me.
- Consent for treatment
- Release of Information
- Consent for video treatment (only if needed)
- Consent for use of email and documentation policies
INSURANCE
I do not contract with any health insurance at this time. I can provide you with a superbill if you have benefits that allow reimbursement to you later under any aspect of your plan.
CANCELLATIONS / NO-SHOWS
The courtesy of at minimum a 24 -hour notice is required to cancel or reschedule appointments. Please call AND email me if your need to cancel. This allows me to offer the time to other clients to take advantage of the available time and helps me use my time efficiently. I do charge for visits at full fee if not cancelled 24 hours in advance.
Other ways to meet: I do offer video conferencing and home visits with existing patients in times of need (for example illness.) A separate consent form must be signed. If you anticipate this might be a need, we should discuss ahead of time to make arrangements.
